Pediatric Associates Inc. has a full time Care Coordinator who helps families with complex healthcare needs. Cases are referred to the Care Coordinator based on diagnosis or by recommendation of the physician.
We use social determinants of health screeners to help the physician know what families might benefit from extra help and connect them to resources in the community.
